Example Client Post-Covid Questionnaire
Homework: Access your PA software account, download questionnaire, select some symptoms based on a complaint you have had, and then use the PA software to create a chart. Search for a symptom, notice the correspondence between “Checklist” in software and the items on questionnaire. Try adjusting the sort order, blinding the client name, looking at the contraindications, scrolling back and forth through the results chart, saving your chart, creating a duplicate consultation to edit differently… play with the software to be ready for Saturday!

Fundamental themes of Ellen is feeling not good enough, feeling disrespected or belittled; consistent digestive issues like bloating, gas, & food “repeating” after eating (“holds onto things” – physically and emotionally). Skin wounds are also very slow to heal yet she also picks at any skin eruption or scab. She feels sad, alone, powerless and punishes herself for being alone while also desiring community. Describes herself as a rebel yet also says she doesn’t like to stand up for herself (“Don’t tell me what to do but tell me what to do!”). All symptoms tend to appear suddenly. Many of her sensations are electrical, like electrical wires. “
